1)* Candidates or RP should use their names identical to that of their ID cards / travel documents / birth certificates for all the entries

2)* For those joining as a group, please submit a Google Sheet (sample) listing all participants’ names and set the General Access to ‘Anyone with the link.’ All names listed should match exactly with their ID cards, travel documents, or birth certificates for all entries

3)* For group participants, please fill in the contact details of the RP

6)* For team, group, or choral participants, please state ‘refer to link of the name list’.

7)* If there isn’t one, please fill ‘None’

11)* Type Chinese title if the participant(s) uses (use) Mandarin or Cantonese.
